Rainier Health Center

Rainier School District is now one of 46 schools in Oregon with a school based health center to improve student health. Having a health center on school campus significantly reduces the amount of class time students miss, and work that parents miss when taking their child to a health care office.

If you would like your child to benefit from Rainier Health Center, download and complete the following forms:

Return the envelope marked "Confidential" to your student's teacher or the district office. Don't forget to seal your envelope!!

If you're unsure about your student accessing Rainier Health Center, please complete as much information as you feel comfortable, and return in a sealed envelope. By including your contact information, we will be able to reach you on a case-by-case basis.
